1. Practice Overview
Aspire Beacon Co. provides physiotherapy services using an integrative and evidence-based approach.
This may include:
Assessment and diagnosis of physical conditions
Manual therapy (joint mobilisation, soft tissue techniques)
Exercise prescription and rehabilitation programs
Movement and postural correction
Education and self-management strategies
Lifestyle considerations relevant to recovery and performance
Where appropriate, we may incorporate a broader perspective including workload, stress, and daily demands to support long-term outcomes.
2. Nature of Physiotherapy
Physiotherapy is a healthcare service aimed at improving movement, reducing pain, and supporting physical function.
Treatment is tailored to the individual and may evolve over time depending on your response and progress.
You acknowledge that:
Outcomes vary between individuals
Improvement may take time and require active participation
No guarantees can be made regarding results
3. Informed Consent to Treatment
By proceeding with physiotherapy services, you provide informed consent to assessment and treatment.
You confirm that:
You understand the nature of physiotherapy
You have had the opportunity to ask questions
You may withdraw consent at any time
4. Risks & Considerations
Physiotherapy is generally safe; however, some risks may include:
Temporary soreness or discomfort
Bruising or inflammation
Fatigue or dizziness
Aggravation of symptoms
Rare risk of injury
You agree to inform your practitioner immediately if you experience any unexpected or concerning symptoms.
5. Integrative Approach
As part of Aspire Beacon Co.’s model, physiotherapy may include consideration of:
Lifestyle factors (sleep, activity levels, workload)
Stress and its impact on the body
Behavioural patterns affecting recovery
Where appropriate, collaboration with other healthcare providers may be recommended.
These approaches support your overall wellbeing but do not replace medical care.
6. Patient Responsibilities
To support safe and effective treatment, you agree to:
Provide accurate and complete medical history
Disclose all relevant conditions, injuries, and medications
Inform your practitioner of any changes in your health
Follow agreed treatment plans and recommendations
Ask questions if anything is unclear
Failure to provide relevant information may impact the safety and effectiveness of your care.
7. Appointments & Cancellations
A minimum of 24 hours’ notice is required to cancel or reschedule.
Cancellations made with less than 24 hours’ notice may incur a fee of up to 50% of the session cost.
Missed appointments (non-attendance without notice) will be charged at 100% of the session cost.
8. Fees & Payment
Payment is required at the completion of each session unless otherwise agreed
Accepted payment methods include card and electronic transfer
You are responsible for any fees not covered by private health insurance or third-party providers
9. Privacy & Confidentiality
Your personal and health information is handled in accordance with the Privacy Act 1988 (Cth) and relevant professional standards.
We:

Store records securely
Do not disclose information without your consent, except where required by law
You may request access to your records at any time.
10. Communication with Other Providers
With your consent, Aspire Beacon Co. may communicate with:
General practitioners
Specialists
Other allied health professionals
This is to support coordinated and effective care.
11. Telehealth (Online Consultations)
Where physiotherapy is delivered via telehealth:
You acknowledge that:
Assessment may be limited compared to in-person sessions
Technical issues may affect the session
You are responsible for ensuring a safe environment
Your practitioner will determine if telehealth is appropriate for your condition.
12. Limitation of Liability
To the extent permitted by law:
Aspire Beacon Co. is not liable for outcomes beyond reasonable clinical control
You acknowledge that results vary and depend on your participation
You accept responsibility for following guidance provided
13. Withdrawal or Discontinuation of Services
You may withdraw consent or discontinue treatment at any time.
Aspire Beacon Co. may also discontinue services if:
Treatment is no longer clinically appropriate
There is non-compliance with agreed recommendations
There are safety concerns

Coaching Agreement & Terms of Engagement
This Agreement is entered into between Aspire Beacon Co. (“Coach”) and the Client.
This document outlines the nature of the coaching relationship, scope of services, expectations, responsibilities, and terms under which coaching services are provided.
1. Description of Coaching
Coaching with Aspire Beacon Co. is a collaborative, client-centred process designed to support clarity, direction, and meaningful progress across areas of life that are important to you.
The coaching process is structured yet flexible, combining reflective conversation, practical strategy, and accountability to help you move forward in a way that is both realistic and sustainable.
Coaching is not about providing answers or prescribing solutions. Instead, it is a process that supports you to:
develop greater awareness of your current situation, patterns, and behaviours
clarify what is most important to you and what you want to move toward
explore different perspectives, options, and ways of thinking
identify practical actions and strategies aligned with your goals
build consistency, confidence, and ownership in your decision-making
The process may involve discussion around both present circumstances and past experiences where relevant, however the primary focus of coaching is forward-looking and action-oriented.
Coaching may address multiple areas of life, including personal wellbeing, professional direction, relationships, lifestyle, and performance. As such, coaching is considered a comprehensive process that may naturally extend across different aspects of your life.
Aspire Beacon Co. integrates a grounded and practical approach to coaching, recognising that sustainable change requires consideration of real-life demands such as time, workload, responsibilities, and energy.
The coaching process is designed to support you in developing your own insights, solutions, and actions. Progress is shaped by your level of engagement, openness, and willingness to apply what is explored during sessions.
2. Scope of Coaching Services
Aspire Beacon Co. provides coaching services through a structured, collaborative process designed to support the Client’s personal growth, wellbeing, clarity, and progress toward agreed outcomes.
Coaching is intended to facilitate the creation and development of personal, professional, health, or lifestyle goals, and to support the Client in developing and carrying out strategies to achieve those goals.
2.1 Life Coaching
Life Coaching is designed to support the Client in gaining clarity, confidence, direction, and alignment in both personal and professional areas of life. This may include:
navigating change, transition, or uncertainty
clarifying goals, priorities, and values
improving self-awareness and decision-making
addressing patterns or behaviours that may be limiting progress
creating practical strategies for moving forward
Life Coaching is future-focused and action-oriented.
2.2 Health Coaching
Health Coaching is designed to support sustainable improvements in wellbeing, lifestyle, and daily habits. This may include:
improving energy, sleep, movement, and stress management
building realistic and sustainable routines
identifying barriers to change and developing practical strategies
strengthening consistency, motivation, and accountability
Health Coaching is educational and supportive in nature and does not diagnose, treat, or cure medical conditions.
2.3 Hypnotherapy-Informed Coaching
Where agreed, coaching may incorporate Conscious Hypnotherapy techniques to support behavioural and mindset shifts. This may include:
reducing unhelpful habits or behavioural patterns
strengthening focus, confidence, and motivation
supporting emotional regulation
reinforcing positive behaviours
These techniques are used as supportive tools within coaching and are not psychotherapy, psychology, or medical treatment.
2.4 Integrated Coaching Approach
Depending on the Client’s goals, Aspire Beacon Co. may draw from one or more of the above coaching areas within the same coaching relationship.
All tools, strategies, and guidance provided are intended to support insight and action, and do not replace advice from qualified medical, allied health, legal, financial, or mental health professionals.
3. Nature of the Coaching Relationship
The Client understands that coaching is a professional relationship focused on personal development, wellbeing, and performance.
The Client acknowledges that:
coaching is not psychotherapy, counselling, or psychological treatment
coaching does not involve the diagnosis or treatment of mental health conditions
coaching is not a substitute for medical, psychological, legal, or financial advice
The Client acknowledges that coaching does not involve the diagnosis or treatment of mental health conditions and is not a substitute for counselling, psychotherapy, or medical treatment.
The Client understands that it is their responsibility to seek appropriate support from qualified professionals where needed.
If the Client is currently under the care of a mental health practitioner, they agree to inform the Coach prior to commencement. In some cases, written consent from that practitioner may be required. The Coach reserves the right to decline or pause coaching services where it is deemed not appropriate or outside the scope of coaching practice.
Coaching may involve multiple areas of life. Decisions about how to apply insights remain the sole responsibility of the Client.
4. Client Responsibility
The Client is fully responsible for their physical, mental, and emotional wellbeing, as well as their decisions, actions, and results arising from the coaching relationship.
The Client agrees to:
communicate honestly and openly
engage with the coaching process with a willingness to reflect and explore
take responsibility for implementing agreed actions
raise questions or concerns in a timely manner
create the time, space, and energy required to participate fully
The Client acknowledges that sustainable change requires commitment, consistency, and follow-through outside of coaching sessions.
5. Coach Responsibility
The Coach agrees to:
maintain the ethics and standards of behaviour established by the International Coach Federation (ICF) (www.coachfederation.org/ethics)
provide a respectful, confidential, and non-judgmental environment
support the Client with structured guidance, tools, and frameworks
work collaboratively with the Client to support their goals
6. Coaching Program & Services
The parties agree to engage in a coaching program as outlined in the Client Pathway Plan (Schedule A).
This may include:
scheduled coaching sessions
agreed communication between sessions
additional tools and resources
Sessions may be conducted in person, online, or via phone.
7. Sessions & Scheduling
Session frequency, duration, and format will be agreed upon in advance.
The Client agrees to attend sessions on time. Late arrival may result in reduced session time.
8. Fees & Payment
Fees are outlined in the Client Pathway Plan.
Payment is required in advance unless otherwise agreed
Payment details will be provided via invoice
Services may be suspended if payment is not received
9. Cancellations & Rescheduling
A minimum of 24 hours’ notice is required to cancel or reschedule an appointment
Cancellations made with less than 24 hours’ notice may incur a fee of up to 50% of the session cost
Missed appointments (non-attendance without notice) will be charged at 100% of the session cost
If the Coach needs to cancel or reschedule, reasonable notice will be provided and the session will be rebooked at a mutually agreed time and date
10. Refund Policy
Refunds are generally not provided for:
missed sessions
late cancellations
change of mind after commencement
Partial refunds may be considered at the discretion of the Coach in exceptional circumstances.
11. Confidentiality
This coaching relationship, as well as all information (documented or verbal) shared by the Client, is treated with respect and confidentiality in alignment with the International Coach Federation (ICF) Code of Ethics.
The Coach agrees not to disclose any information pertaining to the Client without prior written consent. The Client’s identity will not be used as a reference without permission.
The Client understands that coaching is not a legally privileged relationship.
Confidential information does not include information that:
was already in the Coach’s possession prior to disclosure
is generally known to the public
is obtained from a third party without breach of confidentiality
is independently developed without reference to the Client’s information
is required to be disclosed by law, court order, or subpoena
indicates risk of harm to the Client or others
involves illegal activity
The Client agrees to raise any confidentiality concerns in a timely manner.
12. Intellectual Property
All materials, tools, and frameworks provided remain the property of Aspire Beacon Co. and may not be shared or reproduced without consent.
13. Limitation of Liability
The Client accepts full responsibility for their decisions, actions, and outcomes.
Aspire Beacon Co. is not liable for any results or consequences arising from coaching services.
14. Termination
Either party may terminate the coaching relationship with reasonable notice.
Outstanding fees remain payable.
15. Acknowledgment & Agreement
By signing below, the Client confirms that they:
